Certificate Of Insurance

A certificate of insurance is a form used by the insurance industry to provide proof of insurance coverage to an interested third party.

To request a certificate of insurance, fill out the form below and click the "Send Now" button. For Rush Requests, please call 1-888-661-3938 Option #3, between 8 AM and 8 PM Eastern Standard Time Monday through Friday to speak with a Business Insurance Representative.

Policy number may be found in the top right corner of your policy declarations page next to the words "Policy No.". Please input the number as it appears on your policy. A sample policy number is as follows:I-680-317W847-5-COF-98
Please provide the name of your business as it appears on your policy declarations page. The name is found in the upper left portion of the declarations page in the Named Insured and Mailing Address section.

Certificate Type Needed

The certificate holder only wants to verify that coverage exists on the policy. The certificate holder has no insurable interest and is not granted coverage by the policy.
Some certificates are used for a one time event or project whereas other certificates may be provided annually if the certificate holder has a continuing interest (i.e. A company that provides financing for equipment you lease).
The party to whom money or insurance proceeds is to be paid in the event of loss, such as the lienholder on an automobile.

The Certificate Holder has an insurable/financial interest inthe property. The Property IS NOT a building.(e.g. a bank financing an insured vehicle)

The Certificate Holder has requested to be named as an additional insured on the policy for liability purposes.(Adding an additional insured to your policy may change your premium.)

A third party other than the named insured who is covered by the policy. Usually, additional insureds are added to the policy by endorsement. Additional insureds may be referred to in the policy definition of insured. An example of an additional insured is a sub-contractor doing work for a general contractor. The general contractor would be an additional insured on the sub-contractor's policy. There may be additional premium charged when an additional insured is added to the policy.

All coverages listed below that you hold insurance coverage for
A form of insurance designed to protect owners and operators of businesses from a wide variety of liability exposures. These exposures could include liability arising out of accidents resulting from the property or the operations of an insured, products sold by the insured, operations completed by the insured and contractual liability.
Coverage for liability arising out of the operation of your autos.
Coverage for damage to your autos.
Coverage of a physical property for the loss of or loss of ability to produce income.
State mandated coverage of employees by employers
This coverage provides increased limits in excess of the limits on the primary policies. The term umbrella is derived from the fact that it is a separate policy over and above any other basic liability policies the insured may have.
Coverage for things such as your computer equipment, cameras, contractor's tools and equipment.
