  • Include a cover page to indicate who should receive the fax
  • Fax to 800.879.0892

For Electronic Policyholder Report (ePHR) supporting documentation:

  • Log In to ePHR® (use the red Log In box in the top right)
  • Use the cover sheets available under "Access Supporting Documents." Your fax will automatically be associated to your report.
